  • KRS: 0001098382, registration court: Sąd Rejonowy w Bielsku-Białej, VIII Wydział Gospodarczy Krajowego Rejestru Sądowego
  • Tax ID (NIP): 937-27-60-456
  • share capital: 27 000,00 zł (fully paid up)

For any questions about your personal data, email us: [email protected]

What data we collect and why

Through the Keevet Pro pilot programme application form we collect:

  • your full name – so we know how to address you,
  • the name of your practice or clinic (optional) – to prepare a suitable demo environment,
  • your email address and phone number – to contact you and present the system and the terms of the pilot programme,
  • your message (optional) – to answer your questions,
  • the date of your application and the version of the consent text you accepted – so we can demonstrate that consent was given.

Each visit to the website also involves technical data: your IP address and browser information. It is used to deliver the website, keep it secure and protect the form against spam (Cloudflare Turnstile verification and a limit on the number of applications from one IP address).

  • Your consent (Article 6(1)(a) GDPR) – to be contacted about the offer and terms of the pilot programme, given by ticking the box in the form.
  • Steps taken at your request before entering into a contract (Article 6(1)(b) GDPR) – if you decide to join the programme.
  • The controller’s legitimate interest (Article 6(1)(f) GDPR) – responding to your application and contacting a person who reached out to us through the form, as well as keeping the website secure and protecting it against spam and abuse.

Providing your data is voluntary, but we cannot handle your application without it.

How long we keep your data

  • Form data – for 12 months from our last contact, or until you withdraw your consent or successfully object, whichever comes first. If we enter into a contract, the data will be processed for its duration and for the period required by law.
  • Technical server logs – for up to 30 days. They do not contain the content of the form.

Who we share your data with

We do not sell your data or share it for marketing purposes. We use the following processors:

  • Cloudflare, Inc. – handles all traffic to the website (content delivery network, attack protection, secure connection to our servers) and provides the Turnstile spam verification. It processes, among other things, your IP address and browser data.
  • Google (Gmail) – the email service through which we receive applications from the form.

The website and the CRM system (Odoo) in which we manage contacts with applicants run on servers maintained by the controller in Poland. Data stored there is not transferred to an external hosting provider.

Transfers outside the European Economic Area

Cloudflare and Google may also process data in the United States. Such transfers are based on the European Commission’s adequacy decision for the EU-US Data Privacy Framework (for certified organisations) or on standard contractual clauses approved by the European Commission.

Cookies, browser storage and analytics

The website does not set cookies and does not use analytics or advertising tools. Cloudflare’s security mechanisms (including Turnstile) may set strictly necessary technical cookies used solely to protect against bots.

If you choose light or dark mode, we remember your choice in your browser’s storage (localStorage). This information is not sent to us and is not used for tracking.

Automated decisions

We do not make decisions about you based solely on automated processing, including profiling, that produce legal effects or similarly significantly affect you.

Your rights

In connection with the processing of your personal data, you have:

  • the right of access – to find out what data we process about you and receive a copy,
  • the right to rectification – to have outdated or incorrect data corrected,
  • the right to erasure – in particular when the data is no longer needed, you withdraw your consent or you successfully object,
  • the right to restriction of processing – e.g. while the accuracy of the data is being verified,
  • the right to data portability – for data processed on the basis of consent or a contract,
  • the right to withdraw your consent at any time – this does not affect the lawfulness of processing carried out before the withdrawal,
  • the right to object to processing based on the controller’s legitimate interest,
  • the right to lodge a complaint with the President of the Personal Data Protection Office (Prezes Urzędu Ochrony Danych Osobowych, ul. Stawki 2, 00-193 Warszawa, Poland) if you believe that the processing infringes the GDPR.

To exercise your rights, email the address given in the “Data controller” section.
